Opinion Postcard

Fill out this form with the information about the book, magazine, audio, etc., and your opinion. Also, fill in your name and at least one way we can contact you if you win (by phone or by e-mail). Drop this form off at one of the Olathe Public Library Teen Summer Reading Program Desks or email as an attachment to ya@olatheks.org. Winners will be drawn each week during June & July. Final drawing will be held within a week following July 31st. Last day to turn in Opinion Postcards is July 31st.
